Physical Therapist – Travel

Schedule / Job TypeContract | Days | Monday–Friday, 8:30 AM–5:00 PM | One weekend per month required for visits | One major and one minor holiday required | 13-Week Assignment
Key Responsibilities- Provide physical therapy evaluations and treatments to home health patients across the south Lake County coverage area
- Conduct an average of 4–6 patient visits per day
- Participate in a weekend rotation, expected approximately one weekend per month, to support orthopedic cases
- Adhere to the facility's call-off policy, which may result in rescheduling on an alternate weekend or holiday if an absence occurs on a scheduled weekend or holiday
- Maintain accurate and timely clinical documentation in accordance with home health standards
- Travel independently throughout the assigned coverage area using a personal vehicle
- Active Florida Physical Therapist (PT) license required
- Minimum of 1 year of physical therapy experience
- Valid driver's license and current auto insurance required
- SSN and date of birth required for credentialing
- Minimum of 1 professional reference from entire work history
- Prior home health physical therapy experience
- Experience treating orthopedic cases
- Comfort with independent scheduling and self-directed daily workflow in a field-based setting
- Expected 40 hours per week providing consistent, stable work throughout the 13-week contract
- Mileage reimbursement available in accordance with IRS guidelines
