Care Manager

Whitecap Search Healthcare Partners is seeking a dedicated professional to join our team in providing comprehensive support to patients through Health Related Social Needs (HRSN) assessments and navigation services.

 

Job Function:

• Perform Health Related Social Needs (HRSN) assessments and document findings in Unite Us and eCW.
• Provide general navigation services and communicate program objectives to patients.
• Assist with scheduling appointments related to social drivers of health, including reminder and follow up calls.
• Contact patients to support continuity of care and escalate cases requiring enhanced care management.
• Compile and distribute educational materials based on patient needs.
• Identify individual and community resources that support healthy lifestyles (e.g., transportation, exercise programs, community services).
• Maintain timely and accurate documentation of patient interactions.
• Engage approximately 25 new patients per week.
• Perform population management activities as assigned.

 

Requirements:

• Education: High School Diploma (required).
• Language: English (required); Spanish (preferred).
• Work Location: Onsite.
• Demonstrated understanding of health care and social service systems.
• Strong ability to build effective relationships with patients and care teams.
• Excellent interpersonal communication and organizational skills.
• Ability to work independently and collaboratively.
• Strong time management and problem solving skills.
• Patience and capability to navigate complex systems of care.
• Proficient computer skills, with the ability to document concurrently in Unite Us and eCW.
