Company Description Community Hospital-Fairfax is a critical access hospital serving Northwest Missouri with a broad range of inpatient and outpatient services. Role Description This full-time, on-site Licensed Practical Nurse (LPN), Clinic role is based at Community Hospital-Fairfax in Fairfax, MO, however, will service one of our 5 clinics in Tarkio, Rockport, Fairfax, Oregon and Mound City. The LPN will provide direct patient care in the clinic setting, including rooming patients, obtaining vital signs and histories, assisting with examinations and minor procedures, and administering medications and injections as directed by licensed providers. Responsibilities include documenting care accurately in the medical record, coordinating follow-up appointments, patient education, and supporting clinic workflows to maintain timely and efficient patient throughput. The LPN will collaborate closely with physicians, advanced practice providers, and other clinical staff to support high-quality, patient-centered care, while adhering to hospital policies, infection control standards, and safety practices.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Missouri and completion of an accredited practical nursing program.

• Clinical skills in basic patient assessment, vital signs, medication administration, and assistance with exams and minor procedures in an outpatient or clinic setting.

• Strong documentation and electronic health record (EHR) skills, with attention to detail and accuracy in recording patient information.
\n
• Effective communication and interpersonal skills for working with patients, families, and a multidisciplinary care team.
\n
• Ability to prioritize tasks, manage a steady patient flow, and maintain professionalism in a fast-paced clinical environment.
\n
• Commitment to patient safety, infection control, and compliance with regulatory and hospital guidelines.
\n
• Basic life support (BLS) and Advanced cardiac life support (ACLS) certification required; prior experience in a hospital or primary care clinic setting preferred.
\n
• Demonstrated reliability, flexibility with scheduling, and willingness to support the broader needs of the clinic and hospital as needed.
