Role Description This is a full-time, on-site Physical Therapist role based in Santa Clarita, CA, providing services in patients’ homes within the local area. Daily responsibilities include monitoring patient progress, updating documentation, and communicating with physicians, caregivers, and the broader care team to coordinate services. Qualifications

- Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans. \n
- Competence in Patient Care and Home Care, including performing assessments and interventions in varied home environments. \n
- Applied knowledge of Medicine relevant to physical therapy practice, including understanding of diagnoses, contraindications, and care coordination. \n
- Current licensure as a Physical Therapist in the state of California and eligibility to provide home health services. \n
- Experience in home health, outpatient, or similar settings is preferred, with comfort working independently in the field. \n
- Strong communication, documentation, and time-management skills, with the ability to collaborate effectively with interdisciplinary teams. \n
- Valid driver’s license, reliable transportation, and ability to travel to patient homes within the Santa Clarita area. \n
