About this job
Job Description
If you became an FNP because you wanted to know your patients — their families, their histories, their goals — outpatient primary care in Wayne County gives you that. This is a community practice where you see the same families across life stages, and where your work is visible to the people you serve.
The compensation is stated up front: $115,000 to $135,000, with relocation assistance covered for providers coming from outside the area. This is an outpatient primary care role with no remote component.
What makes it different from a typical small-market practice is the backing behind it. The organization has strengthened its affiliation with a larger academic-affiliated healthcare system in recent years — expanding primary and specialty care access, moving to shared clinical systems, and actively recruiting physicians, NPs, and PAs. You get community medicine with real infrastructure behind it.
Why This Opportunity Is Worth a Look- $115,000 – $135,000 base salary, full-time direct hire
- Relocation assistance paid for candidates coming from outside the region
- Outpatient primary care setting — community-focused, relationship-driven practice
- Shared EMR and clinical systems across the affiliated network, supporting continuity when patients move between primary and specialty care
- Expanding access model — the regional affiliation has been used to grow primary and specialty services, not consolidate them
- Northern Finger Lakes location between Rochester and Syracuse, on the southern shore of Lake Ontario
- Provide outpatient primary care to a family patient population across life stages
- Build long-term relationships with patients and families in a close-knit community
- Work collaboratively within a care team connected to a broader regional system
- Document and coordinate care through shared clinical systems used across the affiliated network
- Support preventive care and ongoing management for a population that values accessible care close to home
- Master's degree — Nurse Practitioner
- Board-Certified Family Nurse Practitioner
- Background in primary care, family medicine, internal medicine, community health, or outpatient practice
- Interest in long-term patient relationships and collaborative team-based care
- Willingness to live in or relocate to Wayne County, Geneva, the northern Finger Lakes, or the Rochester/Syracuse corridor
- No visa sponsorship available for this position
Wayne County sits in the northern Finger Lakes on the Lake Ontario shoreline — Erie Canal villages, orchards, farm markets, parks, and festivals. Rochester and Syracuse are both within reach. Geneva and Seneca Lake add waterfront parks, restaurants, wineries, breweries, and trails. If you want small-town pace without giving up access to a city, this region delivers it.
Ready to Explore? If you're an FNP thinking about what your next five or ten years should look like — and whether they should include a patient panel you actually know — this one is worth a conversation. I'm happy to walk you through the practice, the patient population, and the details that don't make it into a posting.
