At Continental Springs, our rehab team is dedicated to helping residents achieve their highest level of function, independence, and quality of life in a subacute/skilled nursing setting. We are seeking a compassionate and motivated

Occupational Therapist to join our interdisciplinary team in a flexible part-time or per diem role.

• Evaluate residents’ functional abilities, including ADLs, mobility, cognition, coordination, and safety

• Develop and implement individualized occupational therapy treatment plans

• Provide therapeutic interventions to improve independence with daily activities and functional mobility

• Address deficits related to strength, range of motion, balance, coordination, cognition, and fine motor skills

• Recommend and train residents in the use of adaptive equipment and compensatory strategies

• Educate residents, caregivers, and staff to promote safety, independence, and carryover of therapy goals

• Collaborate with PT, SLP, nursing, physicians, and other members of the interdisciplinary team to support positive patient outcomes

• Monitor resident progress and modify treatment plans as clinically appropriate

• Maintain timely, accurate, and compliant clinical documentation

• Participate in care planning and interdisciplinary team communication

• Active Occupational Therapist license in Nebraska (or eligibility for licensure)

• Graduate degree in Occupational Therapy from an accredited program

• NBCOT certification preferred

• New graduates and experienced therapists are welcome to apply

• Experience in skilled nursing or subacute rehabilitation preferred

• Strong communication, clinical reasoning, organization, and patient-centered skills
