Position: Full-Time Registered Dietitian
Salary: $65,000 – $80,000 annually

The Hamptons Center for Rehabilitation & Nursing is seeking a compassionate and detail-oriented Registered Dietitian (RD/RDN) to join our interdisciplinary healthcare team. The Registered Dietitian will be responsible for assessing residents' nutritional needs, developing individualized nutrition interventions, and collaborating with clinical staff to promote optimal health and quality of life.

Job Responsibilities

• Conduct initial, quarterly, annual, and significant change nutrition assessments.

• Evaluate current nutrition interventions and recommend appropriate modifications based on residents' individual needs.

• Develop and implement individualized nutrition care plans.

• Provide nutrition education and counseling to residents and their family members.

• Monitor residents' nutritional status, weight trends, dietary intake, and overall progress.

• Collaborate with nursing, medical, dietary, rehabilitation, and other interdisciplinary team members to support optimal resident outcomes.

• Participate in care planning and interdisciplinary meetings as needed.

• Maintain accurate, timely, and compliant clinical documentation.

• Ensure nutritional care is provided in accordance with applicable regulations and facility policies.

• Complete other duties as assigned to support high-quality resident care.

Requirements

• Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution.

• Registered Dietitian/Registered Dietitian Nutritionist (RD/RDN) credential through the Commission on Dietetic Registration.

• RD-eligible candidates are encouraged to apply.

• Nursing home or long-term care experience preferred.

• Experience with MDS, SigmaCare, and Gerimenu preferred but not required; training will be provided for motivated candidates eager to learn.

• Excellent time-management and organizational skills.

• Strong attention to detail and documentation skills.

• Flexible, dependable, and able to work effectively as part of an interdisciplinary team.
