Full Time Speech Language Pathologist/SLP

Synergy Care is looking for a qualified Speech Therapist Pathologist/SLP. We are looking for someone who will provide quality patient care and be a team player. This location is Neuro located near the Space Center.GENERAL STATEMENT OF DUTIES:Uses evidence-based approach to evaluate persons with acquired brain injury and develop an individualized plan of care to reduce impairment, restore function, and increase meaningful participation in life roles. Provide direct services to patients per the plan of care, modify plan of care based on assessment of patient changes, and document services accurately. This role collaborates as part of an intra-disciplinary team leading cognitive, communication, language, dysphagia and when deemed appropriate augmentative alternative communication services. The speech-language pathologist collaborates to facilitate patient rehabilitation and recovery, as well as provide education and training to their care partners. The speech-language pathologist upholds the mission, philosophy and goals of the organization while maintaining strict confidentiality of personnel and patient data and information in the performance of assigned duties.  QualificationsESSENTIAL FUNCTIONS:MAJOR DUTIES AND RESPONSIBILITIES:

• Evaluates patient’s speech, language, cognition, and swallowing abilities and develops a plan of care that is individualized towards the patient’s cognitive-linguistic and/or swallowing capabilities to maximize their functional independence and safety. Integrates comprehensive diagnostic information to differentiate between motor speech, cognitive-communication, language, voice and swallowing disorders.

• Completes appropriate diagnostic assessments via standardized and non-standardized means to create an individualized and patient centered plan of care including recommended frequency and duration of speech therapy services
• Plans and delivers evidence-based intervention based on evaluation and plan of care
• Exercises sound clinical decision making and maintains patient safety when planning, implementing, and evaluating the patient’s response to treatment and evaluation. 
• Plans and delivers education and training so that the patient and/or caregiver understand the patients current cognitive-linguistic and /or swallowing capabilities and how these clinical recommendations can impact the patient’s environmental and safety needs.  
• Consult with and refer to external medical practitioners when needed, in collaboration with the medical director
• Uses evaluation and treatment findings to determine appropriate discharge planning needs
• Evaluates need for equipment and completes justification as necessary for ordering in a timely manner and in collaboration with other clinical and case management staff
• Completes all required speech therapy documentation and billing in accordance with the polices and expectations
• Participates in individualized patient plan meeting and team rounds as warranted by patient need and determined by treatment team

•  Assists with staff education training and competencies as appropriate within licensure
• Actively participates in intra-departmental staff meetings, and intra-discipline/department committee assignments
• Participates as a positive and collaborative member of the interdisciplinary team and inter-program team to ensure patient-centered care
• Adheres to the professional behaviors, expectations, Code of Conduct, Code of Ethics, all Policies and Procedures, and rules and ethics of licensure.
• Responsible for the reporting of employee/patient/visitor injuries or accidents, concerns about abuse or neglect, or other safety issues to the supervisor and in the occurrence notification system.
• Actively participates in center-wide initiatives and staff training that affect occupational therapy practice and scope
• Will maintain necessary certifications to participate in education programs by teaching and/or supervising students based on clinician experience and organizational need. 
• Participates in multidisciplinary competency training to support the transdisciplinary program
• Participates in research projects as warranted by departmental and/or organizational needs.
• Performs other duties as required or requested by supervisor

 MINIMUM QUALIFICATIONS:Required Knowledge, Skills and Abilities 

• General knowledge of computers.
• Demonstrates competency in time management, organization, planning to manage and maintain the demands of patient care caseload and all corresponding job responsibilities.  
• Takes initiative for personal and clinical growth via opportunities for continuing education and maintaining licensure and certification.  
• Collaborates with department, clinical team and organizational support structure and adapting to the changing work demands and environment.  
• Demonstrate NCI abilities and knowledge by successfully passing course, maintain refresher courses, ability to implement the knowledge with patients as needed.
• Maintain CPR certification by successfully passing course and refresher courses as needed per policy.

Education:Master's or Doctoral degree from accredited program in Speech-Language Pathology.Minimum required Certification:

• Texas License in Speech- Language Pathology.
• certificate of clinical competency (Clinical Fellow applicants considered)

 Experience Prefer experience providing direct service to adults with ABI but not requiredIn order to be considered for this position you must have been awarded a master’s degree or higher in Speech-Language Pathology or Communication Disorders or will graduate with such a degree within the next six (6) months. You must have a current American Speech-Language Hearing Association (ASHA) Certification of Clinical Competency in Speech-Language Pathology or be eligible for the certification process by following ASHA and Synergy Care’s guidelines for Clinical Fellowship Year experience.
