At Continental Springs, our rehab team supports residents in a subacute/skilled nursing setting through a combination of on-site collaboration and telehealth services, allowing you to make an impact while maintaining work-life balance.

Responsibilities

• Evaluate and treat residents or on-site services as needed

• Develop and implement individualized physical therapy treatment plans

• Address mobility, strength, balance, endurance, pain management, and functional independence

• Provide patient, caregiver, and staff education to support carryover of therapy goals

• Collaborate with OT, SLP, nursing, and physicians to support patient outcomes

• Maintain timely, accurate, and compliant clinical documentation

• Participate in care planning and interdisciplinary team communication

Qualifications

• Active Physical Therapist license in Nebraska (or eligibility for licensure)

• Graduate of an accredited Physical Therapy program

• Experience in skilled nursing or subacute rehab preferred; new graduates welcome

• Strong communication, clinical reasoning, and patient-centered skills
