Registered Dietitian RD

AXIA Care Center is seeking a knowledgeable and compassionate Registered Dietitian to join our interdisciplinary team full-time. The ideal candidate will be committed to supporting residents’ nutritional health, quality of life, and individualized care in a long-term care and rehabilitation setting.

Key Responsibilities

• Complete comprehensive nutritional assessments and timely follow-up evaluations

• Develop individualized nutrition recommendations and care plans

• Monitor residents experiencing weight changes, altered intake, wounds, renal concerns, tube feeding, or other nutritional risks

• Participate in care-plan meetings and collaborate with Nursing, Dietary Services, physicians, rehabilitation staff, residents, and families

• Review diet orders, supplements, enteral-feeding regimens, laboratory findings, and relevant clinical documentation

• Provide resident, family, and staff education

• Maintain accurate, timely documentation in accordance with facility policies and applicable standards

• Support quality-improvement, survey-readiness, and regulatory-compliance activities

Qualifications

• Current Registered Dietitian

• Bachelor’s degree in nutrition, dietetics, or a related field

• Compliance with all applicable state credentialing requirements

• Long-term care, skilled nursing, rehabilitation, or clinical dietetics experience preferred

• Strong assessment, documentation, organization, and communication skills

• Ability to work collaboratively as part of an interdisciplinary care team
