Physical Therapist (PT) - Home Health Job Overview
We are seeking a skilled Physical Therapist (PT) to join our home health team. The ideal candidate will possess a strong background in home health therapy and will be dedicated to providing high-quality care to patients in their own environments.
Key Responsibilities- Conduct comprehensive patient assessments and evaluations to develop individualized care plans.
- Provide direct physical therapy services in patients' homes, focusing on maximizing the patient’s mobility and functional abilities.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Document patient progress and modify treatment plans as necessary.
- Educate patients and families on treatment plans, exercises, and rehabilitation processes.
- Minimum of 6 months to 1 year of recent home health experience required.
- Must have OASIS documentation experience.
- Current, valid state licensure as a Physical Therapist.
- Ability to travel to patient locations within the designated area.
- Strong communication and interpersonal skills.
Shift: Day (5x8-Hour, 08:00 AM - 04:30 PM)
