Social Services Coordinator

Twin Lakes Extended Care is seeking a Social Services Coordinator to support residents and families throughout the admission, care planning, and discharge process in our skilled nursing facility.

Responsibilities:

• Coordinate and review admissions for appropriate placement

• Complete social service assessments, BIMS, and PHQ-9

• Coordinate discharge planning, including services, equipment, and follow-up care

• Serve as a resource for residents and families

• Participate in care plan meetings and collaborate with the interdisciplinary team

• Maintain advanced directives and accurate documentation in PointClickCare

Qualifications:

• Long-term care or nursing home experience in social services, admissions, or discharge planning

• Knowledge of Medicaid processes

• Degree in social services or a related field preferred

• Strong communication and organizational skills
