Program Coordinator - Partial Care

 

Position Title:Program Coordinator - Partial CareDepartment:Partial CareFTE/Classification:Full-Time / ExemptLocation:125 Atlantic St, Hackensack, NJ 07601Schedule:Monday - Friday (8:30am - 4:30pm; onsite)Salary Range:$65,000 - $75,000 (depending on years of experience and related experience.  

The Program Coordinator of Partial Care is a licensed clinician who assists the Director in general oversight and supervision of all aspects of the program, including clinical and administrative tasks. The Program Coordinator provides coverage for the Clinical Director when they are not present and coordinates the group counseling component of the program. Group Counselors report to the Program Coordinator. The Program Coordinator carries a reduced caseload and may conduct the functions of a counselor and senior counselor including caseload responsibilities, intake and member assignments and oversight of interns.

SUPERVISORY RESPONSIBILITIES:

1. Provides direct supervision to assigned employees to enhance their job skill se
2. Provides on-going support to employees to assure completion of job responsibilities
3. Carries out supervisory responsibilities in accordance with the agency's policies, applicable laws and governmental regulatory agencies
4. Reviews employees' responsibilities to determine time frame and scope of authority, reviews employees' requirements and allotments of available resources
5. Evaluates employees' productivity and documentation of service delivery
6. Responsibilities include interviewing and training employees; planning, assigning and directing work, appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

PERFORMANCE RESPONSIBILITIES: Although each position has its own unique duties and responsibilities, the following list applies to every employee of North Jersey Friendship House:

2. Maintains confidentiality and immediately reports to management breaches of confidentiality
3. Practices job requirements on an accurate, timely and regular basis
4. Demonstrates appropriate professional boundaries with members, co-workers, and supervisors.
5. Meets relevant continuing educational/training requirements
6. Completes all required documentation/paperwork accurately and within established time frames.

Rehabilitation Counselor Responsibilities

1. Develop and regularly review each client's Behavioral Health Care Plan (BHCP), with measurable goals tied to diagnosis, symptoms, and discharge; begin discharge planning at intake and obtain required signatures on time.
2. Complete biopsychosocial and Greenspace assessments at required intervals per regulation and NJFH policy.
3. Build individualized program schedules of daily groups and individual counseling, with group referrals matched to each member's treatment plan.
4. Assess members for pre-vocational training and placement, coordinating with counselors and rehabilitation aides on readiness and fit.
5. Coordinate care with families, the member's circle of support, outside agencies, and medical providers; make referrals to community services and secure all required consents.
6. Maintain accurate, current case records in the EHR (DMHAS, CARF, and MCO compliant), including weekly progress notes and all attendance and required documentation.
7. Attend treatment team and clinical meetings to review cases and address member progress, challenges, and discharge.
8. Manage psychiatric, medical, social, and behavioral crises - providing immediate support, documenting actions, following up, and completing incident reports.
9. Develop comprehensive discharge plans with measurable goals and follow-up services (psychiatry, counseling, vocational, recreational, and others as needed).

Psychosocial Group Program Supervisor

1. Work with the Director to review and improve the content, quality, and strategy of psychoeducational and psychosocial groups, keeping protocols current and evidence-based.
2. Assign group protocol and curriculum revisions to counselors.
3. Audit group notes across all staff and report findings to the Director.
4. Coordinate group coverage and ensure group sizes comply with regulation.

Additional Duties

1. Coordinate and complete intakes, assigning caseloads to counselors.
2. Interview interns and volunteers and coordinate their programs, including educational tours for prospective members and students.
3. Conduct Utilization Review Committee meetings with the Program Director, covering at least 10% of the caseload each quarter.
4. Attend required meetings and complete all assigned training.
5. Complete additional items requested by the Clinical Director or CEO in a timely manner.

REQUIREMENTS:

• Master's degree in rehabilitation counseling, psychology, social work or in another human services
• Licensed in the State of NJ as an LPC, LCSW or Psychologist
• Must have a minimum of 3-5 years of experience in mental health, minimally 1-2 years in a supervisory
