Description
American Traveler is seeking a Home Health Physical Therapist with a minimum of 1 year of recent experience, OASIS proficiency, and HCHB EMR experience to serve a geriatric patient population.
Details
- Home Health setting serving a geriatric patient population
- Day shift schedule, 5x8 hours, 8:00 AM - 4:30 PM
- Average caseload of 5-7 patients per day with a minimum of 30 points weekly
- Territory covers up to a 60-mile radius from the hub office
- On-call rotation may be required
- Uses HCHB (Homecare Homebase) for documentation
Requirements
- Minimum 1 year of recent Home Health experience required
- BLS certification required
- OASIS experience required for staff completing Start of Care assessments
- Proficiency in HCHB EMR required
- Active driver's license and personal transportation required
- License required if applicable
- Skills include home exercise program development and patient/family education
- Experience with orthopedic and neurologic conditions preferred, including post-op recovery, spinal conditions, stroke rehabilitation, and traumatic brain injury
Additional Information
- All charting must be completed by 9:00 AM the following morning
- Timely communication with the care team is mandatory
- Scrubs required (any color), no jeans permitted
- iPad provided; personal cell phone use not required for work
- Mileage reimbursed at the IRS standard rate
- Monday orientation includes completion of billable training modules
- First-time travelers will not be considered for this role
- Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
- Travel pairs accepted, though each traveler is assigned their own territory and must have independent transportation
- Local candidates accepted at the travel rate
- Requested time off must be submitted at the time of application
