Description
American Traveler is seeking a Home Health Physical Therapist with 2+ years of acute hospital and home care experience and certified home health/OASIS proficiency for a 13-week assignment in Pittsfield, MA.
Details
- Home Health setting providing direct patient care in clients' residences
- Day shift, 8-hour shifts from 8:00 AM to 4:30 PM
- Rotating weekends and holiday coverage required if needed
- No on-call requirements
- Uses rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
- 13-week assignment length
Requirements
- MA Physical Therapist license required if applicable
- Active BLS certification (AHA) required
- 2+ years of acute hospital-based nursing care and home care experience required
- Certified home health experience with proficiency in OASIS required
- Valid driver's license required
- Must have own vehicle for home visits
- Must be able to work independently with minimal supervision
- Acute care experience preferred
- Must be able to pass a fit test using an N95 mask (PAPRs not available for home health use)
- One supervisory signed reference from within the past two years required
- Skills checklist for Physical Therapist current within 1 year required
Additional Information
- Responsible for providing physical therapy services and direct patient care to home health clients
- Collaborates with interdisciplinary team members in developing and executing the patient's comprehensive plan of care
- 8 hours of non-billable orientation provided
- Candidates residing within 50 miles of the facility will not be considered
- Mileage reimbursement provided only to/from the TOR office and/or drop/satellite office, not from traveler's home
- No more than 5 days of requested time off will be accepted, and all RTO must be confirmed prior to submission
