Hospice RN Case Manager

The Hospice RN Case Manager plays a vital role in delivering compassionate end-of-life care by assessing patients' needs, coordinating care plans, and managing symptoms within a dedicated hospice team. This nurse case manager handles a typical caseload of 16-25 patients, conducting daily home visits and utilizing the Axxess electronic health record system to ensure thorough documentation and continuity of care.

Responsibilities

• Perform comprehensive patient assessments to determine care needs
• Develop and implement individualized care plans
• Manage medications and monitor treatment effectiveness
• Address and manage patient symptoms to improve comfort
• Advocate for patient needs and preferences
• Coordinate care with interdisciplinary hospice team members
• Maintain accurate and timely documentation in Axxess EHR
• Provide education to patients and families about care processes

Preferred Qualifications

• 2+ years of experience in nursing case management
