Case Manager

The Case Manager will work independently in a support role primarily managing social services cases. This position involves assessing client needs, providing resource referrals, and maintaining detailed case documentation using general office software and the HMIS database system. The Case Manager plays a critical role in offering follow-up support to ensure clients receive the necessary assistance with no travel required.

Required Qualifications

• Minimum 1 year experience in case management
