🏡 Travel Home Health PT – 13 Week Contract | Days

We’re seeking an experienced Physical Therapist (PT) for a rewarding travel opportunity in beautiful Vermont!

📍 Location: Newport, VT
⏳ Contract: 13 Weeks
🕒 Schedule: Monday–Friday | 8:00 AM–5:00 PM
⏰ Guaranteed Hours: 32 hrs/week
💻 EMR: HealthWyse

Requirements:

✅ Active PT License
✅ Minimum 3+ years of PT experience
✅ OASIS experience required
✅ Home Health & Hospice experience required
✅ Experience traveling from patient to patient for home visits
✅ Ability to manage and adapt to changing schedules
✅ Strong communication and documentation skills

Responsibilities:

🔹 Provide physical therapy services in patients’ homes
🔹 Assess patient needs and develop individualized treatment plans
🔹 Deliver appropriate therapeutic interventions to improve mobility and function
🔹 Monitor patient progress and modify care plans as needed
🔹 Educate patients and caregivers on exercises and home-care strategies
🔹 Collaborate with healthcare professionals to support patient outcomes
🔹 Maintain accurate and timely clinical documentation
