Description
American Traveler is seeking a Physical Therapist for a home health assignment in the Fremont, CA area, requiring a current CA PT license and at least 1 year of experience.
Details
- Home health setting covering Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont)
- Day shift schedule
- 13-week assignment length
- Weekend coverage may be required
- Travel outside the assigned area may be required to maintain caseload
Requirements
- Current CA PT license required if applicable
- BLS certification required
- Minimum 1 year of experience required
- Valid driver's license, auto insurance, and auto registration required
- Must provide documentation of TB test (annual), MMR, Varicella, Hepatitis B, Tdap (within 10 years), and annual flu vaccination
- Must pass a drug screen and physical exam
- Must be willing to see COVID-positive and COVID PUI patients
Additional Information
- Responsibilities include performing home visits such as evaluations, re-evaluations, discharges, and OASIS assessments
- Caseload includes a mix of routine visits, evaluations, and OASIS-related visits
- Compensation for visits is calculated using a point-based system that converts to approved hours
- Former employees are not eligible for this travel assignment
- All requested time off must be disclosed prior to assignment confirmation; time off requested after acceptance will not be approved
