Description
American Traveler is seeking a Physical Therapist for a home health assignment requiring an active CA PT license and at least 1 year of experience.
Details
- Home health setting covering assigned territory in Richmond and surrounding areas (Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, Kensington) plus either the Oakland/Berkeley area or the Vallejo/Napa area
- Day shift schedule with possible weekend coverage required
- May require travel outside the assigned service area to maintain a full caseload
- Caseload includes visits such as routine, evaluation, discharge, OASIS SoC/RoC/ReCert, and follow-up visits
- Willingness to see COVID-positive and COVID PUI patients may be required
Requirements
- Current, active CA Physical Therapist license required
- Minimum 1 year of recent experience required
- BLS certification required (license required if applicable)
- Valid driver's license, auto insurance, and auto registration required
- Required immunizations/documents: annual TB test, MMR, Varicella, HepB, Tdap (within 10 years), annual flu vaccine, drug screen, and physical exam
Additional Information
- Responsibilities include performing home health visits, evaluations, re-certifications, and documentation for an assigned patient caseload
- May require travel beyond the primary service area to meet visit expectations
- Former employees of this facility are not eligible for this assignment
- All requested time off must be disclosed prior to assignment confirmation; time off requested after acceptance will not be approved
- First-time travelers may be considered for this role
