Patient Care Coordinator
Location: On-Site
Schedule: 8:30 AM–5:00 PM
Dress Code: Business Casual
Interview: In Person – 2 Rounds

About the Role
We are seeking a Patient Care Coordinator to support daily healthcare operations and provide an excellent patient experience. This role is ideal for a reliable, mature, and efficient healthcare professional with strong communication skills who thrives in a fast-paced environment.
Key Responsibilities
- Check in and check out patients and confirm appointments.
- Collect and update patient demographics and insurance information.
- Perform insurance verification and obtain required patient signatures.
- Schedule appointments, referrals, and specialist follow-ups.
- Answer incoming and outgoing calls professionally and promptly.
- Maintain work queues and front-desk in-basket tasks.
- File, fax, scan, and maintain accurate medical records.
- Monitor fax servers and ensure documents are assigned to the appropriate electronic chart.
- Maintain patient confidentiality and comply with HIPAA, OSHA, and organizational policies.
- Collaborate with Providers, Medical Assistants, Nurses, PSA, Echo Technicians, and other team members.
- Crosstrain and provide coverage at other locations as needed.
