Occupational Therapist In House

Luna Rehab & Care Center is seeking a Part-Time or PRN (consistency) Occupational Therapist to join our in-house rehabilitation team. As an Occupational Therapist (OT), you will evaluate, plan, and deliver individualized treatment using evidence-based approaches to help residents regain independence and improve quality of life. You’ll also play an essential role in patient and family education, interdisciplinary collaboration, and clinical documentation.

• Evaluate residents to identify functional limitations and therapy needs

• Develop and implement personalized treatment plans focused on ADLs and IADLs

• Monitor progress and adjust interventions in collaboration with the care team

• Recommend adaptive equipment and environmental modifications

• Educate patients, families, and caregivers on strategies for functional success

• Maintain accurate and timely documentation per regulatory standards

• Participate in care planning and interdisciplinary meetings

• Ensure compliance with all facility and regulatory guidelines

• Active Occupational Therapist license (NM)

• In good standing with all licensing boards

• SNF or long-term care experience preferred

• Experience with Net Health documentation system is a plus
