Job Summary
Oklahoma Speech Language Pathologist (SNF) This position involves evaluating, rehabilitating, and developing appropriate care plans for patients with communication, cognitive, or swallowing disorders in a skilled nursing facility environment. The role requires utilizing standardized assessments and evidence-based practices to support clinical interventions and ensuring compliance with regulatory and clinical standards.

Job Details

  • Position Description: Effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders, developing appropriate care plans following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Assesses patient progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness. Services are modified based on patient evaluation, and referrals are made in relation to outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers. Communicates effectively with supervisors and team members regarding patient progress, barriers, and treatment plans. Provides a comprehensive discharge summary of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Records treatment minutes accurately. Participates in required meetings, including patient care conferences, utilization review, and rehabilitation meetings. Engages in facility and company trainings, in-services, and conferences. Obtains necessary DME or supplies as per regulatory and clinical practice requirements. Supports and supervises CFY or Student Speech-Language Pathologists. Adheres to ASHA and State practice acts, code of conduct, and ethics during professional practice.
  • Required Qualifications: Certificate of Clinical Competence (CCC) certified Speech-Language Pathologist (SLP), Oklahoma State License, 1 year of SLP experience, LTC or Rehab experience, Level II experience (not just Level I observation), and ability to supervise/support CFY or Student SLPs.
  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
  • Submission Requirements: Must possess active ASHA and State Licensure.
