Description
American Traveler is seeking a Home Health Physical Therapist with 2+ years of home health experience for a 13-week travel assignment.
Details
- Home Health care setting serving patients of all ages with multiple comorbidities
- Caseload of 4-5 patients per 8-hour day
- Documentation must be completed within 1 day of patient visit
- Uses HCHB (HomeCare HomeBase) EMR
- Schedule is Monday-Friday, 8am-5pm, 5x8-hour day shifts
- Support from PTAs/COTAs and 24/7 manager availability
- Mileage reimbursed per GSA rate
Requirements
- Minimum 2 years of PT experience, with prior home health experience required
- Current BLS certification required
- CA PT license required if applicable
- Valid driver's license and car insurance required
- Active state or compact license required at time of submission; pending licenses not accepted
Additional Information
- Responsibilities include standard physical therapy treatment and timely clinical documentation
- All MyJourney training modules must be completed prior to arrival; travelers may be delayed if incomplete
- Copy of driver's license required for consideration
- Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and rehire eligibility
- Facility may cancel up to 4 shifts per 13-week assignment
