What You’ll Do

As the MDS Coordinator, you will oversee and coordinate the Resident Assessment Instrument (RAI) process while working closely with the interdisciplinary team to ensure accurate, timely, and compliant assessments and care plans.

Responsibilities include:

• Coordinate and participate in the completion of the MDS, CAAs, and Care Plans in accordance with current federal and state regulations.

• Collaborate with the interdisciplinary team to ensure accurate and timely completion of resident assessments.

• Develop and communicate MDS assessment schedules for all applicable payer sources.

• Communicate changes to the MDS assessment schedule to all appropriate interdisciplinary team members.

• Lead and/or participate in daily PPS meetings, weekly Medicare meetings, and month-end meetings to support regulatory compliance and accurate billing.

• Gather and verify information required for MDS completion through medical records, bedside assessments, and interviews with residents, families, and staff.

• Develop and monitor systems to ensure interdisciplinary team members complete, date, and sign required assessments in accordance with federal regulations.

• Observe direct nursing care, review clinical documentation, and make appropriate recommendations.

• Assist with chart reviews and clinical audits to promote accuracy, compliance, and quality of care.

• Work collaboratively with nursing and interdisciplinary team members to identify changes in resident condition and ensure care plans appropriately reflect residents' needs.

What You’ll Need

• Current, active RN or LPN license in North Carolina.

• Minimum of 3 years of clinical experience in a healthcare setting; long-term care experience preferred.

• Previous MDS/RAI experience strongly preferred.

• Current, active CPR certification.

• Strong knowledge of nursing and medical practices, procedures, and standards of care.

• Knowledge of federal and state regulations, guidelines, and requirements related to long-term care.

• Strong organizational, communication, and documentation skills.

• Ability to work effectively with residents, families, nursing staff, and the interdisciplinary team.

• Professionalism, personal integrity, attention to detail, and a commitment to quality resident care.
