MDS Coordinator

Full Job Description

Edgewood Manor of Wellston is seeking an MDS Coordinator to join our team! This position is responsible for coordinating the care delivery interventions for residents in their assigned area in accordance with current professional practice standards, physician's orders, facility policies and procedures, and local, state, and federal regulations and within the scope of practice. Must have knowledge of PDPM. MDS Nurse is responsible for completion of MDS Assessments and development of resident care plans. Registered Nurse preferred. MDS experience of 1 year also preferred.
