Director of Rehabilitation (DOR)

About the Role:
The Director of Rehabilitation (DOR) is responsible for the overall management, leadership, and clinical excellence of the rehabilitation department within the skilled nursing facility. This role oversees physical therapy, occupational therapy, and speech-language pathology services to ensure high-quality, patient-centered care that meets regulatory, clinical, and financial standards.
Key Responsibilities:Clinical & Program Oversight
Direct and coordinate all rehabilitation services (PT, OT, SLP)
Ensure compliance with federal, state, and local regulations (CMS, Medicare, Medicaid, PDPM)
Maintain high clinical standards and evidence-based practice
Monitor patient outcomes, progress, and discharge planning
Collaborate with interdisciplinary team to optimize resident care
Leadership & Staff Management
Recruit, hire, train, supervise, and evaluate rehab staff
Provide mentorship, coaching, and performance feedback
Ensure adequate staffing levels to meet census and productivity goals
Promote a positive, collaborative team culture
Operational & Financial Management
Manage department budget, productivity standards, and labor costs
Track and analyze key performance indicators (KPI’s)
Ensure accurate documentation and billing practices
Participate in facility census development and case mix optimization
Compliance & Documentation
Ensure timely and accurate clinical documentation
Oversee audits and respond to survey findings related to rehab services
Maintain licensure, certifications, and continuing education compliance
Communication & Collaboration
Serve as liaison between therapy department, nursing, physicians, families, and administration
Participate in care conferences, quality assurance meetings, and leadership meetings
Educate staff and stakeholders on rehabilitation services and outcomes
Required
Licensed Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP)
Active, unrestricted state license
Minimum of 2–3 years of experience in a SNF or post-acute setting
Previous leadership or supervisory experience
Preferred
Director of Rehab or Rehab Manager experience
Strong knowledge of PDPM and Medicare regulations
Experience with EMR systems and rehab management software
Strong leadership and team-building skills
Excellent communication and organizational abilities
Clinical expertise in geriatric and post-acute rehabilitation
Ability to analyze data and manage productivity
Problem-solving and decision-making skills
