Job Title: Healthy Start Advanced Practice Clinician (LPC / LCSW) ( Family Support Coordinator)
Job Summary Under general direction, the Healthy Start Advanced Practice Clinician (APC) serves as a key member of the San Antonio Healthy Start grant leadership team alongside the Program Manager, Social Services Coordinator, and Coalition Coordinator. This position provides clinical support, individual and family therapy, and program co-leadership to achieve grant deliverables. Responsibilities include delivering direct clinical services—with a focus on maternal, infant, child, and family health, as well as trauma and abuse recovery—co-facilitating community initiatives, developing program procedures, and representing the program with high-level collaborators. Exercises direct supervision over assigned staff.
Essential Job Functions
- Clinical Services & Case Management
- Provide direct clinical services, assessments, screenings, and individual/family therapy (primarily focused on maternal/child health, family dynamics, coping skills, and trauma/abuse recovery).
- Develop and implement individual- and family-centered treatment plans; conduct counseling in office or home settings as needed.
- Facilitate client support groups, educational initiatives, and case conferences with clients and staff.
- Program Leadership & Administration
- Serve as co-lead for the Healthy Start consortium and its grant-related activities, ensuring required deliverables and metrics are met.
- Develop, evaluate, and update program procedures, abstracts, poster presentations, and educational materials for clients and families.
- Administer and review client paperwork, maintaining updated records and completing monthly reports on service hours and client characteristics.
- Community Engagement & Workforce Development
- Serve as a program liaison and representative to clinical partners, high-level collaborators, and community stakeholders.
- Deliver health education, presentations, and training for community participants, staff, and the broader healthcare workforce.
- Connect clients to City, State, Federal, and private resources while networking across social service, healthcare, law enforcement, and criminal justice systems.
Job Requirements
- Education: Master’s degree from an accredited college or university (no substitution allowed).
- Licensure: Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) under Texas State Licensing Laws (or licensed in another state with the ability to obtain Texas licensure within 60 days).
- Experience: Two (2) years of increasingly responsible experience in individual, family, and group therapy, including one (1) year of direct counseling experience with children and families affected by trauma or abuse.
- Background Check: Must pass and maintain a Criminal Justice Information Systems (CJIS) fingerprint-based background check. Disqualifiers include felony convictions/deferred adjudications, Class A & B misdemeanor deferred adjudications, Class B misdemeanor convictions, open arrests, and family violence convictions.
- Licensing/Certification: Valid Class "C" Texas Driver's License.
Knowledge, Skills, and Abilities
- Knowledge of counseling principles, systematic-oriented therapy, case management, and child/family trauma dynamics.
- Knowledge of laws involving child abuse, family conflicts, and local/regional social service agencies.
- Ability to establish, evaluate, and modify clinical action plans and programmatic procedures.
- Ability to lead, co-facilitate, and network with multi-disciplinary stakeholders, clinical collaborators, and public health systems.
- Ability to communicate clearly and effectively, handle high-stress situations (including substance abuse dynamics), and operate standard office/computer equipment.
