\n WCS Healthcare Partners is seeking a skilled Physical Therapist to join our Home Health team. This role requires expertise in providing comprehensive physical therapy services to a diverse patient population, including adults and geriatrics, within a home health setting. The ideal candidate will have a strong background in orthopedic, neurological, and cardiopulmonary care, along with a commitment to patient education and safety.Responsibilities\n

• Conduct thorough assessments of patients to develop individualized treatment plans.
\n
• Implement therapeutic interventions to improve patient mobility, strength, and functionality.
\n
• Educate patients and families on home exercise programs and safety protocols.
\n
• Utilize adaptive devices and equipment such as crutches, walkers, and gait belts effectively.
\n
• Monitor patient progress and adjust treatment plans as necessary.
\n
• Maintain accurate and timely documentation of patient care and progress.
\n
• Collaborate with other healthcare professionals to ensure comprehensive patient care.
\n
• Provide education on signs and symptoms of physical, sexual, and psychological abuse.
\n
• Ensure compliance with all safety and injury prevention protocols.
\n

Requirements\n

\n
• Minimum of 2 years of experience in physical therapy, with home health experience preferred.
\n
• Active Florida or Compact license required at the time of submission.
\n
• Basic Life Support (BLS) certification required.
\n
• Experience with OASIS and Cerner systems preferred.
\n
• Strong skills in assessment, documentation, and patient/family education.
\n
• Ability to manage a patient population that includes cardiopulmonary, geriatrics, neuro, ortho, and oncology.
\n
• Must have a valid driver's license and reliable transportation for home visits.
\n
• COVID-19 vaccination required or an exemption form on file.
\n
• Flu shot required or declination with mandatory mask wearing at work.
\n
