Position Overview\n

We are seeking a qualified Level 1 AP Primary Care professional to provide high-quality primary care services in a clinical healthcare setting. The successful candidate will support patient assessment, diagnosis, treatment planning, health education, and ongoing management of patients while working collaboratively with other healthcare professionals.

Key Responsibilities\n
    \n
  • Provide primary care services to patients in accordance with established clinical standards and protocols.
  • \n
  • Perform patient assessments, obtain medical histories, and evaluate presenting symptoms.
  • \n
  • Develop appropriate treatment plans based on patient needs and clinical findings.
  • \n
  • Diagnose and manage common acute and chronic health conditions within scope of practice.
  • \n
  • Order and interpret appropriate diagnostic tests and laboratory results.
  • \n
  • Prescribe medications and treatments as permitted by applicable licensure and organizational guidelines.
  • \n
  • Provide preventive care, health screenings, patient education, and wellness counseling.
  • \n
  • Monitor patient progress and modify treatment plans when clinically appropriate.
  • \n
  • Maintain accurate and timely patient documentation and electronic medical records.
  • \n
  • Coordinate patient care and make referrals to specialists or other healthcare services when needed.
  • \n
  • Collaborate effectively with physicians, nurses, allied health professionals, and other members of the care team.
  • \n
  • Follow applicable clinical, regulatory, safety, and infection-control requirements.
  • \n
  • Maintain patient confidentiality and comply with HIPAA and organizational policies.
  • \n
Required Qualifications\n
    \n
  • Appropriate advanced-practice clinical education and credentials for the assigned role.
  • \n
  • Current and unrestricted professional license appropriate to practice primary care in the applicable jurisdiction.
  • \n
  • Relevant primary care/clinical experience.
  • \n
  • Strong clinical assessment, communication, and patient-care skills.
  • \n
  • Ability to work effectively as part of a multidisciplinary healthcare team.
  • \n
  • Proficiency with electronic medical records and clinical documentation.
  • \n
  • Ability to provide compassionate, patient-centered care.
  • \n
Preferred Qualifications\n

  • Experience in a primary care or outpatient clinical environment.

  • Experience managing both acute and chronic patient conditions.
