We are seeking a full-time Quality Engagement Specialist to join our Patient-Centered Medical Home team. This role supports quality-improvement initiatives by engaging with patients and families, coordinating outreach, assisting with care-gap closure, and collaborating with clinical and administrative team members to improve health outcomes and the overall patient experience.
Job Summary:
The Quality Engagement Specialist is a position that performs a wide array of administrative tasks to support the Quality and Compliance Department's approach towards evaluating and/or improving patient engagement.
Specific Duties and Responsibilities:
Collaborates with QI team members to establish quality standards, benchmarks, and measures of performance in support of Agency Programs/Departments.
- Patient outreach as required to attain quality goals and measures, including daily phone recalls to schedule appointments required by insurance plans and CMC requirements.
- Compiles data from various software systems and departments for data retrieval, analysis, performance improvement monitoring, and strategic planning.
- Assists in running, managing, and data review processes for assuring accuracy and integrity of health services data reports to meet regulatory and operational requirements.
- Creates data tools for recording objective assessment information for performance management and development process.
- Participates on Comprehensive Review Team for internal audit readiness related to program performance initiatives, qualitative standards, and contract compliance.
- Participates in other committee meetings to support QI initiatives.
- Follows instructions for recording, evaluating, and reporting quality and reliable data.
- Maintains professional appearance and neat work habits.
- Participates in ongoing skills development.
Knowledge, Skills, and Abilities:
- Exemplifies excellent customer service, verbal communication skills, conflict resolution with patients/parents/guardians, visitors, and other employees; shows courtesy, friendliness, helpfulness, and respect.
- Experience working with patients and appointment scheduling in an electronic health record is preferred.
- Strong organizational skills and a demonstrated ability to maintain accurate notes and records.
- Strong phone, spelling and grammar, computer, and keyboarding skills.
- Strong interpersonal skills and understanding and commitment to delivery of patient-centered medical care with a team-based approach.
- Ability to work independently, exercise creativity, and is detail oriented.
- Ability to manage multiple and simultaneous responsibilities.
- Consistently alters plans/routines when situation requires and continues to perform without projecting stress/frustration that would adversely affect the work environment.
- Maintains and ensures patient privacy and confidentiality.
- Takes the initiative to proactively assist Receptionists with or without direct supervision and to resolve problems.
Job Qualifications:
- Education: Minimum high school diploma or equivalent.
- Clinical Experience Preferred.
- Proficiency with Microsoft Outlook, Excel, and Power Point preferred.
