Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team.

 

Location: Parsippany, NJ 07054

 

Weekly Gross Pay: Up to $2,750

 

Schedule: Monday–Friday, 8:00 AM–4:30 PM

Weekly Hours: 40 hours

Productivity: 30 points/week

Coverage Area: Morris County

On-Call/Weekend: One weekend per month

Mileage: Reimbursed between patient visits

Patient Setting: Home Health

Key Responsibilities

  • Provide physical therapy services to patients in their homes.
  • Evaluate patient needs and develop individualized physical therapy plans of care.
  • Deliver treatment to improve mobility, function, and independence.
  • Monitor patient progress and communicate with physicians and the healthcare team.
  • Complete timely and accurate patient documentation and visit notes.
  • Meet productivity expectations and participate in the monthly weekend rotation.

Requirements

  • Active Physical Therapist (PT) license.
  • Home Health experience required.
  • CPR certification.
  • Valid driver's license, auto insurance, and reliable vehicle.
  • Strong communication, clinical assessment, and care planning skills.
