Job Description
MUST HAVE 5-6 YEARS HOME HEALTH CARE EXPERIANCE.
Responsibilities:

- Coordinate and monitor patient care, ensuring that all necessary medical services and resources are provided
- Collaborate with healthcare professionals, including doctors, nurses, and therapists, to ensure effective communication and coordination of care
- Advocate for patients' rights and ensure that they receive appropriate medical treatment and support
- Monitor and evaluate the effectiveness of treatment plans and make adjustments as necessary
- Maintain accurate and up-to-date medical records for each patient
Skills:
