### Requirements:

✅ Active California Physical Therapist License
✅ Minimum 3 Year of Physical Therapist Experience
✅ Current BLS Certification
✅ Valid Driver's License, Auto Insurance & Vehicle Registration
✅ Home Health Experience Preferred
✅ Willingness to travel throughout assigned service areas

### Responsibilities:

✔ Conduct Physical Therapy Evaluations and Re-Evaluations
✔ Develop and Implement Individualized Treatment Plans
✔ Complete OASIS Documentation and Assessments
✔ Educate Patients and Families on Therapy Goals and Home Exercises
✔ Collaborate with Interdisciplinary Healthcare Teams
✔ Maintain Accurate and Timely Documentation
