Are you a compassionate and dependable Certified Nursing Assistant who is passionate about providing exceptional resident care? Hopedale Medical Complex is seeking a dedicated Certified Nursing Assistant (CNA) to join our Nursing Home team.

In this role, you will provide hands-on care to residents while working alongside an experienced interdisciplinary team committed to quality, compassionate, and resident-centered care. You will assist residents with activities of daily living, monitor and report changes in condition, and help maintain a safe, comfortable, and respectful environment.

This position is primarily based in the HMC Nursing Home. Cross-training and occasional floating to the Hospital and Commons Assisted Living Facility may be required based on resident care needs and staffing requirements.

Essential Duties and Responsibilities

Resident Care

  • Provide compassionate, high-quality direct resident care under the supervision of an RN or LPN.
  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, mobility, and meals.
  • Assist residents with positioning, transfers, ambulation, and other mobility needs while maintaining safety and comfort.
  • Follow individualized care plans and nursing instructions.
  • Assist with maintaining resident comfort, dignity, privacy, and independence.
  • Observe residents and promptly report changes in condition, behavior, or other concerns to licensed nursing staff.
  • Assist with maintaining a clean, safe, and comfortable resident environment.
  • Provide care in accordance with established infection prevention and safety practices.

Teamwork and Communication

  • Communicate effectively with residents, families, nursing staff, and other members of the interdisciplinary care team.
  • Work collaboratively with RNs, LPNs, and other healthcare professionals to support quality resident care.
  • Demonstrate professionalism, dependability, compassion, and respect in interactions with residents, families, and coworkers.
  • Respond appropriately to resident needs and changes in condition.
  • Maintain resident confidentiality and comply with HIPAA requirements.

Professional Responsibilities

  • Maintain compliance with HMC policies, procedures, and resident safety standards.
  • Participate in required in-service training, continuing education, competency assessments, and other professional development activities.
  • Demonstrate a commitment to quality improvement and resident-centered care.
  • Maintain current knowledge and skills necessary to perform assigned duties safely and effectively.
  • Cross-train and float to other HMC departments or facilities as assigned.
  • Perform other duties as assigned.
Required Qualifications
  • High school diploma or equivalent required.
  • Current Illinois Certified Nursing Assistant (CNA) certification required.
  • Current CPR certification required.
  • Excellent communication and customer service skills.
  • Compassionate and respectful approach to resident care.
  • Ability to work effectively as part of an interdisciplinary healthcare team.
  • Ability to follow nursing instructions, individualized care plans, and established policies and procedures.
  • Strong attention to detail and ability to recognize and report changes in resident condition.
  • Ability to maintain resident confidentiality and comply with HIPAA requirements.
  • Ability to perform the essential functions of the position with or without reasonable accommodation.
  • Willingness to cross-train and float to other Hopedale Medical Complex departments and facilities as needed.
Preferred Qualifications
  • Previous experience working in a nursing home, long-term care, assisted living, hospital, or other healthcare setting.
  • Experience providing direct care to older adults or individuals with complex healthcare needs.
  • Previous experience working as part of an interdisciplinary healthcare team.
Physical Requirements

The physical demands described below are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodations may be made to enable qualified individuals with disabilities to perform these essential functions.

  • Constantly (67–100% of the time): Standing, walking, talking, hearing, near visual acuity, and use of hands and fingers for resident care, documentation, and operation of equipment.
  • Frequently (34–66% of the time): Reaching, bending, stooping, pushing, pulling, repositioning residents, assisting with transfers, and assisting residents with mobility.
  • Occasionally (11–33% of the time): Lifting and carrying items weighing up to 50 pounds and assisting with resident transfers using appropriate body mechanics, team assistance, and mechanical lifting equipment when available.
  • Rarely (less than 10% of the time): Climbing ladders or working at heights.

Additional physical requirements include:

  • Ability to safely assist residents with transfers, repositioning, ambulation, and other mobility needs.
  • Ability to respond promptly to resident needs and emergency situations.
  • Ability to perform repetitive movements associated with resident care, documentation, and use of equipment.
  • Ability to work with residents who may have physical limitations, cognitive impairment, illness, injury, or emotional distress.
  • Ability to safely perform the essential functions of the position with or without reasonable accommodation.
Work Environment

Work is performed primarily in the HMC Nursing Home in a resident-care environment. The employee will have frequent direct contact with residents, families, nursing staff, and other healthcare professionals.

The position may involve exposure to infectious diseases, bodily fluids, cleaning products, and other healthcare-related hazards. Appropriate personal protective equipment (PPE) will be provided and must be used in accordance with established policies and procedures.

Cross-training and occasional floating to the Hospital and Commons Assisted Living Facility may be required based on resident care needs and staffing requirements.
