Role Description This is a full-time, on-site Physical Therapist role based in Clinton, MO. The Physical Therapist will assess patients’ functional status, mobility, and pain levels, and develop individualized treatment plans aligned with home health and hospice goals. Daily responsibilities include delivering evidence-based therapeutic interventions, educating patients and caregivers on exercises and safety, and monitoring progress to adjust care plans as needed. The role involves close collaboration with nurses, physicians, and other interdisciplinary team members to ensure coordinated, goal-oriented care. The Physical Therapist will also complete timely documentation, participate in case conferences, and support quality and compliance standards.

Qualifications

\n

\n
• Licensure and clinical preparation: Active Physical Therapist license in the state of Missouri
\n
• Clinical and technical skills: Strong assessment, treatment planning, and therapeutic intervention skills, with the ability to manage a varied caseload in home health settings.
\n
• Communication and collaboration: Excellent verbal and written communication abilities, including educating patients and families, and effectively collaborating with an interdisciplinary care team.
\n
• Organizational and documentation skills: Proficiency in maintaining accurate clinical documentation, managing time and schedules, and using electronic health records or related software.
\n
• Personal qualities and experience: Demonstrated empathy, professionalism, and cultural sensitivity, with a valid driver’s license, reliable transportation, and willingness to travel locally. Prior home health or hospice experience is preferred but not required.
