Medical Social Worker

Role Description

This is a full-time on-site role for a Medical Social Worker located in Kingman, AZ. The Medical Social Worker will be responsible for providing social work services to patients and families, including but not limited to discharge planning, case management, and social services. The Medical Social Worker will collaborate with interdisciplinary teams, assess patient needs, develop and implement care plans, and provide counseling and support to patients and families.

• Experience in discharge planning and case management

• Knowledge of community resources and social services
\n
• Excellent communication and interpersonal skills

• Ability to work independently and collaboratively in interdisciplinary teams

• Experience in hospice or home health is a plus

• A Bachelor's or Master's Degree in Social Work or related field
