Responsibilities:

• Provide personal care services, including bathing, dressing, grooming, and toileting, to our clients

• Assist with daily living activities, such as feeding, transferring, and ambulating

• Monitor and report changes in clients' condition to the nursing team

• Maintain accurate and detailed records of client care and services provided

• Work collaboratively with our multidisciplinary care team to develop and implement personalized care plans

• Demonstrate a commitment to excellent customer service, respect, and dignity for our clients and their families
