Description
American Traveler is seeking a Physical Therapist for a Home Health position requiring 1+ years of home health experience and knowledge of HCHB and OASIS.
Details
- Home Health setting
- Day shift, 5x8s, Monday through Friday, no weekends
- No call required
- Mileage reimbursed per government rate
Requirements
- Doctor of Physical Therapy (DPT) degree required if licensed on or after January 2016, or foreign equivalent
- Active PT license required in state of practice (license required if applicable)
- Current BLS/CPR certification required
- Valid driver's license required; must be at least 21 years of age
- Minimum 1 year of home health experience required
- EPIC experience required
- Knowledge of HCHB and OASIS required
Additional Information
- Plans and conducts treatment programs to restore and improve physical, social, and mental functions
- Serves as co-leader in an interdisciplinary approach to patient care
- Completes evaluations, establishes individualized treatment and discharge plans, and communicates plans to appropriate parties
- Completes comprehensive and accurate clinical documentation within established deadlines
- Supervises assigned assistants
- Required vaccinations are a condition of employment, with medical and religious exemptions available
- Accepts local candidates with no radius rule; local rates match travel rates
