Warm Valley Health Center is seeking a skilled and compassionate Clinical Nurse/Case Manager to join our fast-paced outpatient team in Fort Washakie, WY.  What You'll Do:

• Perform patient triage, assessments, and administer urgent care treatments

• Deliver medications, IV therapy, and assist in minor procedures

• Conduct EKGs, lab draws, and prepare patients for diagnostic exams

• Coordinate care plans and referrals with an interdisciplinary team

• Provide chronic care support, case management, and patient follow-up

• Lead wellness initiatives and community health education outreach

• Maintain accurate and timely EHR documentation, ensuring compliance

 What You Bring:

• BSN required

• Active Wyoming RN license

• BLS certification (ACLS/PALS preferred)

• Experience in outpatient, urgent care, or case management

• Certification in Case Management is a plus (or willingness to obtain within 1 year)
