Community Support Specialist

Job Summary

We are looking for a compassionate and reliable Community Support Specialist to empower adults with developmental and intellectual disabilities. In this direct-care role, you will serve as a guide and liaison, helping clients navigate daily life, engage in local community activities, and advocate for their personal choices and rights.

Key Responsibilities

• Skill Building & Independence: Teach and assist clients with daily living skills, such as meal preparation, budgeting, personal care, and household management.
• Community Integration: Accompany and transport clients safely to community outings, social events, medical appointments, and employment or vocational sites.
• Person-Centered Planning: Collaborate with families, case managers, and healthcare teams to implement individualized support plans (PSPs/IEPs) and track goal progress.
• Health & Medication Management: Properly record and administer medication according to medical protocols, and monitor general health and wellness.
• Advocacy & Support: Offer compassionate emotional support, act as a strong client advocate, and assist in crisis de-escalation or conflict resolution.
• Documentation: Maintain accurate daily logs of activities, resident behaviors, and milestone achievements.

Qualifications & Requirements

• Education: High school diploma or GED required; associate's or bachelor's degree in psychology, social work, or a related human services field is preferred.
• Experience: Minimum of 1 year of experience working with individuals with special needs, developmental disabilities, or in a direct-care/human services setting.
• Licensing & Certifications:
• • Valid local driver’s license with a clean driving record.
  • CPR/First Aid certification (or willingness to obtain upon hire).
  • Medication administration certification (e.g., CMT/MTTP) if required by state or facility guidelines.
• Skills: Strong interpersonal communication, active listening, patience, empathy, and basic computer proficiency for case note documentation.

Physical & Work Environment

• Ability to lift/move up to certain weight thresholds, assist with mobility support, and spend extended time standing or walking.
• Background checks, drug screenings, and motor vehicle record checks are mandatory conditions of employment.
