What We're Looking For

• Bachelor's degree in Social Work, Human Services, Human Resources, or a related field; applicable experience may be considered based on position requirements.
• Social Work licensure, including CSW or LCSW, preferred where applicable.
• Four or more years of progressive HR experience and/or relevant healthcare experience preferred.
• Previous experience in skilled nursing, healthcare, long-term care, or a multi-site environment strongly preferred.
• Experience working with electronic medical records (EMR) and/or HR information systems.
• Strong interpersonal skills with the ability to build trusted relationships with residents, families, employees, and leadership.
• Excellent organizational skills with strong attention to detail and the ability to manage multiple priorities.
• Clear, professional verbal and written communication skills.
• Ability to maintain strict confidentiality and appropriately handle sensitive resident and employee information.
• Proficiency with basic computer applications, including Microsoft Word and Excel.
• Ability to read, speak, and understand English and accurately complete required documentation.
• Ability to meet the physical demands of the position, including extended sitting or standing, adequate vision and hearing, and lifting, pushing, or pulling as required.

What You Will Do

• Support residents and families with social service needs, care planning, discharge planning, resources, and access to outside services.
• Complete social service assessments, MDS documentation, care plans, interventions, measurable goals, and other required EMR documentation.
• Maintain communication with residents and families regarding health updates, care needs, available resources, and discharge planning.
• Assist residents with financial, legal, and community resources, including Social Security, VA benefits, transportation, voting assistance, and preplanning services.
• Participate in interdisciplinary care plan conferences and assist with resolving resident, family, or ombudsman concerns and grievances.
• Promote resident dignity, individuality, confidentiality, and positive relationships between residents, families, and staff.
• Lead and support HR Representatives by providing training, guidance, policy interpretation, and assistance with day-to-day HR processes.
• Maintain and audit employee records, personnel files, and HR processes to support accuracy, consistency, and regulatory compliance across facilities.
• Support onboarding and employment eligibility processes, including E-Verify, and partner with payroll to help ensure accurate and timely processing.
• Administer employee benefits and leave programs, including enrollments, deductions, COBRA, open enrollment, FMLA tracking, and required documentation.
• Manage employment verifications, garnishments, benefit invoice reconciliation, compliance requests, and other HR-related documentation.
• Identify opportunities to improve and standardize HR workflows, contribute to HR initiatives and projects, and support a positive workplace culture.
