Support Community Connections/Respite

Support Community Connections/Respite role involves providing essential community support and respite care services to clients in various settings, including community centers and client homes. As an individual contributor, this position requires frequent travel and focuses on client advocacy, crisis intervention, and effective family communication to promote well-being and community integration.

Responsibilities

• Deliver community support and respite care to clients
• Engaging with clients either in the home or outside of the home
• Respond promptly to crises and de-escalate situations
• Maintain clear and compassionate communication with families
• Document all interactions and services provided accurately

Preferred Qualifications

• 1+ year experience in caring for individuals with disability
• High School Diploma or equivalent
• Strong active listening and empathy skills
• Experience with crisis management and conflict resolution
• Effective interpersonal communication abilities
• Proficient documentation skills
• Cultural competency to engage diverse communities
