Outpatient Referral Coordinator
Pay: $16.00 - $19.00 per hour
Job description:
Physicians' Office - Monday - Friday 8:00AM - 5:00PM
We are seeking a full-time, experienced Referral Coordinator to support providers with obtaining patient referrals.
Responsibilities include:
- Working closely with providers, nurses, and patient/families to complete referrals.
- Contacting other physicians' offices and facilities regarding referrals
- Processing patient referrals using EHR and insurance portal
Key Success Factors:
- Experience with processing patient referrals
- Strong attention to detail
- Skill in the use of computers and related software applications
- Strong verbal and written communication skills
- Critical thinking and complex problem-solving skills
- Multi-tasking
- Ability to work with others and independently

Total Care 2 U is a local medical practice run by doctors dedicated to providing comprehensive primary and chronic/transitional care to patients in the comfort of their homes. Additionally, we coordinate services with home health agencies, hospitalists, assisted living, independent living, and CCRs.
