Your Role as an Occupational Therapist

As an Occupational Therapist, you will provide skilled, individualized therapy services designed to help residents improve functional abilities, participate in meaningful daily activities, and achieve greater independence.

Key Responsibilities

• Evaluate referred residents and determine their rehabilitation needs.

• Assess medical condition, functional capabilities, limitations, restrictions, and rehabilitation potential.

• Develop and implement individualized treatment plans based on each resident’s needs, abilities, capacity, and tolerance.

• Establish realistic, measurable, and achievable therapeutic goals.

• Provide skilled occupational therapy services in accordance with physician orders and established plans of care.

• Monitor resident progress and modify treatment approaches as clinically appropriate.

• Educate residents and families on therapeutic techniques, adaptive strategies, safety recommendations, and ways to support independence.

• Complete accurate and timely evaluations, weekly progress notes, re-certifications, treatment documentation, and discharge summaries.

• Collaborate with nursing, physicians, rehabilitation staff, and other disciplines to provide coordinated, patient-centered care.

• Maintain confidentiality and comply with HIPAA, Federal Residents’ Rights, and applicable regulatory requirements.

• Perform other duties as assigned by the Director of Rehabilitation.

Qualifications & Requirements

• Graduate of an accredited Occupational Therapy program.

• Current and valid Occupational Therapist license/registration in the state of practice.

• Strong clinical, communication, documentation, and organizational skills.

• Compassionate, patient-focused approach to resident care.

• Ability to work effectively as part of an interdisciplinary team.

• Long-term care and/or geriatric rehabilitation experience preferred, but not required.
