Case Coordinator

We are looking for a compassionate and organized Case Coordinator to join our home care team. The ideal candidate is comfortable traveling to client homes and working directly with clients, families, and care providers.

Responsibilities

• Conduct client assessments and develop individualized care plans.
• Coordinate home care services, appointments, and communication with clients, families, and providers.
• Monitor client progress and update care plans as needed.
• Maintain accurate documentation 
• Provide emergency direct care coverage when staffing needs arise.
• Communicate effectively with clients, families, caregivers, and other service providers.

Qualifications

• With healthcare experience; home care or case management experience preferred.
• Experience working with individuals with disabilities is a plus.
• Current CPR/First Aid certification.
• Strong communication, organization, and problem-solving skills.
• Reliable transportation and willingness to travel to client homes.
• Ability to work independently and manage multiple client needs.
