Description

American Traveler is seeking a Home Health Physical Therapist to serve adult and geriatric patients across South Carolina counties, requiring at least 1 year of experience and SC licensure or compact eligibility.

Details

• Home health setting covering Barnwell, Aiken, and Edgefield counties in South Carolina, with most patients located in Aiken or Barnwell County
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• No weekend shifts required
• Average daily census of 175 with approximately 6 visits per day and full-time productivity around 25 units
• Mileage reimbursed at IRS standard rate and tracked through HCHB/Epic
• Floating required within facility and scope of practice as needed
• Uses Home Care Home Base (HCHB) for charting, with transition to Epic anticipated around September 2026
• Clinicians without HCHB experience must complete an online course prior to starting

Requirements

• Minimum 1 year of experience required; candidates with 6 months may be considered though 2 years is preferred
• BLS certification required
• South Carolina state license or compact PT license required if applicable
• Experience with joint replacements, COPD, CHF, and Type 2 diabetes management preferred
• Experience with home health documentation including OASIS, evaluations, and care plans
• Skilled in assessment, gait training, therapeutic exercise, and use of assistive/adaptive devices
• Comfortable treating orthopedic, cardiopulmonary, and neurologic patient populations
• First-time travelers accepted

Additional Information

• Responsibilities include home-based evaluation, treatment, and documentation for adult and geriatric patients
• Scheduler works to group patient visits geographically to minimize travel time and improve efficiency
• Typical daily driving distance ranges from 20-30 miles, with a maximum of about 70 miles
• Any scrub color accepted
• Maximum RTO of 7 days per contract, and must be finalized at time of submission
• No RTO permitted during orientation week
• If 1-2 major holidays fall during the assignment, no holiday RTO can be requested; if 3 major holidays occur, one holiday RTO request may be considered
• Major holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving/Black Friday, Christmas Eve/Day, and New Year's Eve/Day
• Local candidates accepted at the same bill rate
• Returning Piedmont employees must be away from all Piedmont facilities for one full year before applying as a traveler
