Our Care Manager position plays a vital role in assessing client needs, coordinating services, connecting clients to appropriate community resources, and ensures required documentation is completed accurately and on time. Acting as a central advocate, this role bridges the gap between clients, their families, and partner organizations. The ideal candidate possesses exceptional organization and a compassionate approach to service delivery.

Key Responsibilities

· Conduct face-to-face evaluations and assessments.

· Develop and maintain individualized, person-centered care plans.

· Coordinate services and assist individuals with essential services.

· Monitor service delivery and ensure compliance with program guidelines.

· Serve as the lead advocate and care coordinator for all clients.

· Maintain confidentiality as related to patient information.

· Communicates with new and existing clients, family members, client representatives and referral sources to ensure the highest quality of services is provided.

· Conducts client quality calls with new and existing clients and provides follow up as to family members, caregivers and other relevant persons for each case as needed.

· Makes sure that all client introductory and supplemental visits are scheduled and take place; schedules the Meet and Greet/introductory visits for the management team to introduce caregivers when they are starting a new client case.

· Maintains complete, accurate and timely records.

· Required to participate in an on-call rotation, once every 3 weeks, and every 3rd holiday.

· Complies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the organization.

Minimum Qualifications

· Associate's degree in Social Work, Nursing, or a related field is preferred.

· Minimum 2 years of experience in home and community-based services, or related field.

· Experience working with individuals with developmental disabilities is preferred.

· Strong knowledge of the home care industry and local community resources.

· Must have a valid driver's license.
