The RN, Coord, Cardiovascular collaborates with other professional disciplines/unit leaders to ensure safe and effective patient care delivery and the achievement of desires patient outcomes for the Cardias population. She/he is responsible for facilitation of nursing practice and continuum of care outcomes, in collaboration with the bedside nurse, ancillary practitioners, and physician and case manager. The RN, Coord, Cardiovascular coordinates patients' care from pre-admission, hospitalization, to post-discharge follow-up in collaboration with the healthcare team, to best meet the patients' physical, emotional, and spiritual needs. The RN, Coord, Cardiovascular uses the nursing process that includes assessment, problem identification, goal definition, plan development, evaluation, reassessment and plan revision, and documentation in the patient record to achieve optimum patient outcomes on both an individual and population basis.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
