Job Description
Job Summary:
Homeside Rehab, a premier home health care provider, is seeking a compassionate and skilled Occupational Therapist or Occupational Therapy Assistant (OTA) to join our home health team. As an Occupational Therapist or OTA, you will play a vital role in helping patients reach their full potential by delivering individualized, evidence-based therapy in their own homes.
Job Responsibilities:
Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
Provide evidence-based OT interventions to improve motor skills, functional independence, and daily living outcomes
Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
Collaborate with the clinical team to ensure coordinated and effective treatment
Communicate clearly with office staff, patients, and caregivers regarding progress and care plans
Educate patients and caregivers on strategies to support continued progress between sessions
Adapt treatment approaches to meet the needs of patients with diverse diagnoses, ages, and functional levels
Travel to patients' homes within the assigned service area, maintaining a flexible and reliable schedule
Job Qualifications:
Active NYS Occupational Therapy License (OT or COTA) required
Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program (OT), or completion of an accredited OTA program (COTA)
Prior home health experience preferred; new graduates are welcome to apply
Basic computer proficiency for electronic documentation
Valid driver's license and reliable transportation
Strong communication and interpersonal skills, with the ability to build rapport with patients and families
Ability to work independently while collaborating effectively with a multidisciplinary team
